Necrotizing Fascitis of Perineum

Abstract

|
During a 3 and 8/12 year period, eight patients were treated for necrotizing fascitis of the perineum. These infections were caused by anorectal abscess, illicit injection for hemorrhoids, fish-bone impaction, and spontaneously.
It begins with fascial necrosis at onset and rapidly progress to surrounding fascial planes, eventually involving overlying skin and underlying muscle. Skin changes were present in all patients and fascial necrosis was confirmed in 5 patients. Crepitus was found in 5 patie-nts and extended up into the lower abdominal wall.
E. coli was the most predominant organism and E. coli with Enterococcus the most common combination.
Operation was often delayed for several days because of the difficulty in recognizing and presence or wide fascial necrosis.
Prompt extensive surgical debridement is of prime importance in treatment of necrotiziog fascitis of the perineum.
